The Petition for Writ of Certiorari and the Writ of Certiorari should name the Division Administrator as the respondent and should be served on the Division of Hearings and Appeals at:

    5005 University Avenue, Suite 201
    Madison, WI 53705-5400

The Division will prepare and file the record with the court only upon receipt of the court order.
